How can I get a Health Certificate for work?

You may submit the following requirements listed below to the Health Office and then pay the required fee at the City Treasurer’s Office. For first time applicants attend the HIV Seminar at Velarde Health Center. Present the official receipt to the Health Office and wait for your document to be release.

*You can only avail this service if you are employed in the City of Imus.

  • Results of Fecalysis

How to apply for a Mayor’s Certification?

  • Proof of attendance in an orientation seminar conducted by Philippine Statistics Authority for Solemnizing Officers

How can I get married at Imus City Hall?

You may submit the following requirements listed below to the Mayor’s Office and then pay the corresponding fee/s at the City Treasurer’s Office. Present the official receipt to the Mayor’s Office and wait for the wedding orientation and confirmation that you suggested dates are available. If you have preferred venue, seek the approval of your chosen officiate first.

Find at least two people within the legal age who will serve as your witnesses. A parent or a guardian is required if either of the parties are under the legal age of 18. During your wedding day, you need to pay a filing fee. This is to enable them to forward their own duplicate of the marriage contract to the local civil registrar.

How can I request for Financial Assistance?

You may submit your request letter and the following requirements listed below to the Mayor’s Office. After the request has been evaluated and processed, present the official receipt to the City Treasurer’s Office and wait for your check to be release.

Requirement(s):

  • Duly Accomplished Client Information Form
  • Financial Assistance letter addressed to Hon. Emmanuel L. Maliksi
  • Photocopy of a Valid ID

For Trainings/Seminars/Competitions:

  • Invitation with the date, time and venue of the event
  • Training Design and appropriations of any event

For Sportsfest/League:

  • List of players and coaches
  • Certification from the Barangay Captain attesting their participation

For Homeowners’ Association:

  • Board Resolution stating the request
  • List of officers

For Barangay:

  • Barangay Resolution

Note: An individual or entity may be granted of financial assistance only once every quarter as long as their purpose is within the guidelines or subject for evaluation.

How can I request for Scholarship Assistance?

You may submit your request letter and the following requirements listed below to the Mayor’s Office. After the request has been evaluated and processed, present the official receipt to the City Treasurer’s Office and wait for your check to be release.

  • Duly Accomplished Scholarship Information/Evaluation Form;
  • Letter Request addressed to the Mayor;
  • Latest Report Card not lower than 85% (High School)/Certification of Grades (College);
  • Present Registration/Enrollment Card (with assessment of tuition fees);
  • School Certification of Good Moral Character (Original Copy);
  • Certification from Barangay (Original Copy);
  • CSWDO Certification of Indigency of parents/guardian (Original Copy);
  • Birth Certificate (Photo copy);
  • Photocopy of Voter’s Registration or COMELEC ID of Parents and student aged 18y/o above;
